Broken axles

Obviously this was a serious failire on any locomotive. I do not remember any specific tales of them on the Rio Grande narrow gauge, but I am sure they happened. I vaguely remember it something like that may have happened on the D&S, but I might be wrong. Broken axles are signs of bigger problems,like wheels out of quarter, wheelsets out of tram, mismatched rods, a crown brass that burned up, or abuse, like spinning the wheels too much, or pulling loads right at or above rated tonnage. I know that snow fighting was very hard on locomotives, especially the frames.
