I don't know, but I'd guess either the RGS or the scrappers (who were using #42). I seem to remember that #42 was used for bucking snow, and the damage probably occurred then.

The crude repairs (well, actually I don't know if they're crude or not - visually they looked rough to me, but I'm no expert on such things) were probably made by RGS crews, I'm guessing. I don't think any of it occurred after she left the RGS, but then again I'm not sure.

I do have photos and a pretty good history of #42 after it left the RGS, near the bottom of my loco timeline:
