This ought to help:
A "T" junction on the pole (the "Break Iron" Ed mentioned) is all that's needed. The earth wire could drop through the desk and building floor, or go back out to the pole and be earthed there.
That said, you can indeed loop the wire into the building and put the "T" inside, which has the advantage of an inside test point. Get out your Delorean, visit enough 1930-era stations and you'll probably find both designs in place.
