The lead engine starts off, gently pulling the slack and getting the whole shebang going, if possible. After all is started, or if the leader is about to stall, the second engine starts adding his power. At least that's the way B&O/CS&X do it. I'm talking about first through third generation diesels here; a pair of K's on some mountain grade might need to do something different. You just don't want to add too much power at once to jerk a drawhead or knuckle.
