Telegraph poles would be planted in hand-dug holes. I spent a summer on the Craig Branch stubbing and replacing poles. You might be surprised at how quickly a hole can be dug by two guys with a banjo and a spoon. Then the gang would go from hole to hole, working together to raise and plant the new pole.
