Amen! Steam railroading needs some sort of centralized Safety Reporting System, such as the Aviation Safety Reporting System (ASRS) that is run by NASA. ASRS provides a forum for pilots, maintainers and controllers to submit reports detailing their experiences, so that others can learn from them and hopefully avoid repeating some of the nasty ones.
The monthly summary publication for ASRS is called "Callback" and is usually a pretty interesting read.
