The linked report of the accident investigation on the WP&Y in 2006 should be required reading for all those involved in operating and managing a railroad. In a nutshell there were contributing factors to the accident including crew training, equipment malfunctions, and operating procedures which ultimately resulted in one crew member losing his life and 3 others seriously injured. The attached photo from Russ Sperry taken recently shows a similar operation on the C&TS. It apparently also shows a lack of car capacity data on the JBWX similar to the WP&Y gondolas involved in the accident, which was a contributing factor (overloading of the cars with ballast) in the runaway.
