If you look at the pictures you can get a good idea of how it all fits together. The piston is driven/sweated onto the rod. In this case the rod broke off just inside the slide. From the picture of the broken end you can see that it had obviously been cracked for a LONG time, as there is a lot of oxidation, and only a narrow shiny streak in the center. It would appear that some vertical looseness in the slide may have been the last straw for it.
