This would be the opportunity to center score (notch) the cylinder head studs (if they are not now) so if the cylinder head is blown off the studs will break in their center and not pull a chunk of the cylinder casting. Is hammer tapping the piston Key a daily requirement, to see if they are tight, by the shop men, or engineer? EDIT: Piston rod Key was not loose as the cause was a ring failure jamming in a steam port causing the piston casting to shatter. Track rails break, rings break, no employee failure here. Thanks to Taylor's post below. D&S is a great finely operated and interesting railroad. Things happen.
