This all brings up some questions on how the gauge is measured. I had always assumed that it is the distance from the inside edges of the heads of the rails.
This would make since since the head different rail weights may be different widths.

For standard gauge, what is the distance between the outside edges wheel flanges?
Same question for 3' gauge.
