Actually a barrel failure. A dry pipe failure is when the engine takes off at full power even though the throttle is closed. Southern #4501 suffered a dry pipe failure in the 1970's.

The crew's actions or the boiler water level have nothing to do with barrel failures. They are the result of fatigue cracking or other mechanical defects the the barrel course, and are the reason behind the FRA requirement to remove the tubes every 1,472 service days (or every year if it is a lap seam boiler) in order to be able to closely inspect the health of the barrel.
