So did they just say (oh foo) and weld up the holes that were missed?

Seriously Mr. Ed, you have a person inside the boiler to line up the tubes on the fire box end and one in or in front of the smoke box to stab the tubes in from that end. As Linn and Earl said, they have to coordinate very carefully. It helps to establish a pattern of rows and columns.
