Hi mate...
Traditionally they ran as would a standard steam loco - "tender" at the rear containing coal and water and the tank at the front of the smokebox carrying water. The set up helps distribute weight and given the wheel arangement are equally adept at running in either direction.
As an aside, there is also running a narrow gauge 2'6" G42 garratt on the Puffing Billy line in Victoria, Oz
I am eagerly waiting on the load trials to see it stretch its legs after all this time...
