"Smothered" usually means that it's slathered with green chili, often with lettuce and tomato on the side. Actual ingredients may vary according to the whims of the chef and the customs of the region. Scoville (heat) index of the chili can range from mild to "torture". I find that after the first couple bites, it usually doesn't much matter. You nerve endings get cauterized and the afterburn doesn't feel so bad. Doesn't mean that you may not pay later.
