They do indeed apply an actual load to a bridge to see what happens. What happens is picked up by several load sensors and the data is used to construct a load path model. They look for anomalies in that model. They compare the measured model of actual loading to the calculated load paths, which are based on strength of materials. In the case of the fire-heated iron, they would also do sampling and testing of the iron and reconcile that finding against the load path model.
