The C&S had a special accessory for just that sort of thing...it was called a 2"x8" (or something like that). I tend to recall one photo of a C&S (or maybe DL&G) engine in which this accessory was either not completely successful, or forgotten, as a chunk of the glass is missing.
I wonder a bit about how the range of a headlight would be impacted by the location...higher isn't necessarily better. The center of the cone of light for a high mounted headlight would be above cars at grade crossings, and its illumination of track level would start further out (not that it really matters for most railroads). I'd guess that the visors on many headlights helped with both of these aspects.
Personally, I think the smart railroads took cosmetics into account
