The White Pass had a trainman fatality while switching at Skagway in July 2000. A train or engine was being backed into a storage track (maybe around a curve) the conductor -- an experienced man -- was using a radio; conductor must have tripped and got hit by the train. See

Knott's Berry Farm had a death of a trainman in an end-of-the-day switching incident back in October, 1998. It was the first employee death for the park.
