I had considerable luck finding records for Eureka through the office of the chief boiler inspector for the states where it ran. They were very helpful. You would be surprised what they might have in their archived files. While they may or may not have form 4s they just might have a lot of other calculations that may be very helpful.
Also, check with some of the historical societies where the locomotive may have run. Often they wind up with all kinds of business and mechanical records of various railroads. It may take a lot of work to sift through all the stuff, but sometimes you do find that nugget you're looking for....ie the form 4.
