I don't know how many reading this have filled a tender from a water spout. When the water blasts out of the spout when a normal height water tower is full, there is considerable recoil that forces the person on the tender to straddle the spout to hold it down or at least hold it with both hands along with the chain to the flapper. Imagine the pressure out of this baby in the photo, it might have taken the fireman, engineer and the conductor to keep the spout in the tank!
