There were two other steam monorails I am aware of: the Sonoma Valley Prismodial and the Epson Salts Railway.
Beyond that, I think the narrowest non-park/live steam/excursion/etc. railway in the US was 2'-0" on common carrier and 18" on some tram/mine railroads.
