I agree with all points stated above this. I’ve trained for years in confined and enclosed space regulations. Like all OSHA regulations, they seem overreaching and not necessary but time and again we are shown why there are rules and regulations. Commonsense is not a bench mark.
I am reminded (even haunted) by the images of a neighboring construction site. Nine fire trucks. Four ambulances. Many police, sheriff and emergency supervisor cars. One medic helicopter. They pulled two lifeless bodies out of the new storm sewer and one still alive but died later at the hospital. When asked why these men were not following confined space rules, the company replied, “this is the way we have been doing it for 30 years and we have never had an issue until that day”.
Simply put, rules are written down because at some point there was a failure in this type of work. Many times it resulted in death.
