Bill, like this, lay track to edge of water course, drive abutment piles if any, otherwise set abutment sills and caps, lay stringers out over said abutment, lay track on top, advance piledriver and drive piles in, repeat, rinse and repeat again until water course crossed....

Take a look at any Bent Diagram such as this [
www.drgw.net] now note that the Batter angle isn't very far off centre at commencement, outer piles were dragged into position at batter angle with a crab winch in the streambed in my experience on a Work Train.
