A little follow-up. The name relates to the wheel arrangement, not the frame placement. So there were inside frame 2-8-2's like Uintah 40, and outside frame 2-8-2's like Uintah 30, but both are mikados. The name "midado" frequently got shortened to "mike' in common useage. After December 7, 1941 a number of American railroads decided that 'mikado' was too Japanese, and re-named thier 2-8-2's "MacArthurs" - the Union Pacific being the most prominent western example I can think of. In this regard these roads followed the same pattern shown by the American public in 1917 , when sauerkraut became 'liberty cabbage, and hamburger became 'victory steak.'
