Speaking of Damaged Rotaries

This question has been on my mind for some time. After reading about the RGS single rotary being severly damaged by a boiler explosion. It got me to wondering. What were the causes leading up to the rotaries demise? Was it carelessness, lack of water, or mechanical malfunction?
