The rule using steps on a locomotives is to replace the old footboards. Footboards were included on a locomotive until the 1970's, and were right in line with the wheelsets. I have seen some tourist lines that have put stirrups on the end of the pilot beams in order to comply with the footboard prohibition as well.

As far as riding cars go, there is no rule in place to prohibit anyone from riding side ladders, stirrups, etc. Because of slack action the riding of car ENDS is prohibited.
