It's a CB&Q 5-Chime. The C&S got a lot of them due to leased engines from CB&Q. I don't believe the CB&Q "designed" that whistle, but I have only seen them on Q power. Maybe they got it from some manufacturer and made their own patterns and produced their own. A lot of roads did that. A CB&Q whistle is easily recognized by the "Bowl". It has a flange at the bottom and has a very durastic taper up to the top.
The picture is one the one on CB&Q O5-A 5633.
