Im not sure of the track configuration (main vs diverging...) but, I see an electric lock on the one switch and that farther switch has a green target when lined against this flow of traffic in the picture. Perhaps this is a cross over between to main yard tracks and they shouldnt have one lined main and one diverging? If that is the third item, someone probably was operating the switch on hand throw due to the derailed car. Hard to tell without being qualified.
