The story I have been told is 473 lost the crosshead/piston rod key above Elk Park. The piston rod and crosshead parted company and punched the right piston into the head, shattering the head. The rod then proceeded to seat itself back into the cross head. They were either trying to move the piston/rod ahead in the cylinder thinking the rod would easilly pop loose, or move the engine slightly, with the noted results.

Fortunately, the rod and piston were not damaged.
