At SVRR we originally used ASME safety valves on both engines as we were under state jurisdiction from the beginning up to about 2000. They did not stay in adjustment and were prone to unexpected failure. The incident I am thinking of many years ago occurred with our #3 Heisler. We were finishing the annual inspection and steam test with the FRA present. Both ASME valves had just been returned from being repaired and adjusted by a code repair outfit. #3 is legal to 180psi it's original pressure, however we have long had the lower safety at 150 and of course the upper 155. We got up to 150 and the lower safety did not lift, we continued on up to 155 and neither safety was lifting. When we got to 160 I started an injector and cooled it down. The FRA man just said something like, "well I guess you'll get that taken care of". We did. We bought a new Consolidated copy from Strasburg for the lower one and put on a perfectly good non ASME Lunkenheimer for the upper one. 19 has the same set up. The Consolidated copies they make work very well.
