During my carrier in the tourist railroad industry, I had to experience the loss of 2 friends who were killed while performing their duties in that industry. Since then, I have had to investigate other fatalities along with observing numerous safety and operating rules violations that are leading indicators as to the next accident or future fatality. It is up to the railroad and their employees/volunteers, from the top down on how they approach safety, identify risks and then eliminate or mitigate those risks.
