The extra spike hole could have been made at the time of manufacture, but a more likely scenario is that it was repunched at a later date. On the SVRR all of our tie plates came from various UP branch lines, and many of them have been repunched more than once. You can tell this because the spacing is very uneven, with some holes almost overlapping.
