bridge guard rails

I have noticed that both with Narrow gauge and with Std gauge RRs, the placement of guard rails while crossing trestles/bridges seems to vary in practice. Usually I see two rails, one each protecting the main rail. But often I have seen pictures where only one safety rail is in place; ususally on the inside. Is there any true standards, or was this each RR's choice or depended on availability of materials. Likewise Is there a standard the the size of the guard rail with respect to the mainline.
