Unlike on a shay where all three cylinders are in a line, that engine's center cylinder is cocked at an angle so when it's "square" it won't sound like it.

The crank pins on the wheels are 120 degrees apart, as opposed to 90 as is usual. The center crank is offset from an nominal 120 degrees by the same amount as the angle of the center cylinder.

So the center cylinder exhaust event will be closer in timing to one of the outside cylinders than the other making for the uneven exhaust in the video.
