Sanders on steam locomotives

I am curious how the dry sand in the sand dome is applied when needed.
Was it an a pressurized system using steam or air to blow the sand down on to the rails?
Or was it simply a gravity feed?
Was there more than one method used and it simply depended on the likes and dislikes of the CMO of a particular railroad?
How was the sand feed process activated?
