As always, good advice from Chris.
Find a copy of the National Boiler Inspection Code (NBIC) and read 49 CFR Part 230.

These documents should start helping you with the general questions. I can answer the basic questions regarding Part 230 while others (Chris, Linn, Kelly and more) can help with the same and more technical items.
