On Rio Grande narrow gauge Caboose's a brake wheel will set up the brakes on both trucks. Just like on a coach with 2 brake wheels.

Why are their 2 brake wheels, one on each end? I don't think there is one answer. Could be so you can tie it down from what ever end is most convenient. If you are doing something like a flying switch and you are riding the car ready on the hand brake you may want to be on the end facing the direction of travel.
