For the record one of the locomotives in the Toltec Gorge head-on collision in 1922 was running on one side only. The 411 with engineer Lee Smith and fireman Ralph McGuire was running light east back to Alamosa when it collided with Train 115, killing engineer Dave Wright and fireman LJ Knee. The cause was determined to be that Smith and McGuire didn't follow train orders however Smith may have been so intent on running his crippled engine that he blocked the details of orders out of his mind.
